What is going on

Sciatica is a symptom, not a diagnosis.

Sciatica is the pain, pins and needles or weakness that runs from your lower back into the buttock and down the leg when the sciatic nerve or its roots are irritated. The important part is finding what is irritating it, because the cause is what we treat, not the label. In most people the nerve is being compressed or sensitised near the spine, and it calms down over time with the right approach.

The good news is that most sciatica settles without surgery or injections. A small number of presentations do need to be seen sooner, such as rapidly progressing weakness or changes in bladder or bowel control. Screening for those is part of your first appointment, so you are never left guessing.

How we treat it

Settle the nerve, then load it back up.

1

Find the source and screen it properly

Your physiotherapist works out where the nerve is being irritated and which movements calm it versus aggravate it, and screens for the red flags that need a doctor. You get a clear explanation of what is driving the leg pain.

2

Settle the symptoms

Nerve-friendly movement, positioning, and hands-on treatment where appropriate to take the heat out of the nerve and get the leg pain retreating back toward the spine, which is the sign it is settling.

3

Rebuild with strength

Once symptoms allow, our exercise physiologist rebuilds strength and tolerance through the back, hips and legs so the nerve is not being provoked again. Physio and exercise physiology on the same plan, one clinic.

4

Back to sitting, lifting and training

We progress your tolerance for the things that used to set it off, whether that is long drives, desk work, lifting, or getting back on the mats, and tell you honestly when you are ready.

Causes we see
  • Disc-related nerve root irritation
  • Referred pain from the lower back
  • Nerve sensitivity after a lifting injury
  • Buttock and deep hip-related symptoms
  • Sciatica that flares with sitting or driving
  • Recurring nerve pain that keeps returning

Common Questions

Before you ask.

Will I need surgery?

Most sciatica settles without surgery. The nerve calms and the leg symptoms ease over weeks with the right movement and load. Surgery is considered only in a minority of cases, and we tell you which signs mean you should be seen sooner by a doctor.

How long does it take to settle?

Many cases improve over a few weeks, though nerve pain can be slower and come in waves. As the leg pain retreats back up toward the spine, that is a good sign it is settling, and we progress your plan accordingly.

Can I exercise with sciatica?

Usually yes, with the right movements. Some positions calm the nerve and some aggravate it. We prescribe nerve-friendly movement first, then rebuild strength as symptoms allow.

Do I need a scan or referral?

No referral is needed to book as a private patient, and scans are not needed for most sciatica. We screen for the red flags that do warrant urgent imaging or a doctor.

Can I claim on private health or Medicare?

Yes. Private health extras with HICAPS on the spot, Medicare rebates with a GP care plan, and WorkCover for work-related cases.
