For Referrers
Referring a patient
is simple.
We currently have capacity for new patients. Referred patients are onboarded at the earliest available appointment, and you receive correspondence after the initial assessment.
We accept referrals from GPs, specialists and allied health for exercise physiology and physiotherapy. Every referred patient gets a structured initial assessment. You get a letter after that assessment, and a summary at discharge.
What to refer- Musculoskeletal injuriesBack, neck, shoulder, hip and knee, acute or persistent
- Sports & training injuriesAssessment, rehab and staged return to sport
- Work-related injuriesActive claims, with return-to-work planning and reporting
- Persistent pain & deconditioningPatients needing structured, supervised exercise rather than passive care
- Chronic conditions where exercise is indicatedIncluding type 2 diabetes, cardiovascular and metabolic conditions
How it works, start to finish
- You send the referral, or the patient contacts us directly
- We contact the patient and arrange the first appointment
- Structured initial assessment with the treating clinician
- You receive an assessment letter with the diagnosis and plan
- Active block of care, then a discharge summary back to you
To refer, email us with:
- Patient name, DOB and contact details
- Presenting condition and relevant history
- Any imaging or specialist reports
- CDM plan paperwork, if applicable
- Claim number and insurer details, for workers compensation or CTP
